Today, we would like to share a case

in which a tooth that had previously received

root canal treatment and a crown

developed inflammation,

and we performed re-root canal treatment

at our clinic.

The patient we would like to introduce

visited our clinic

complaining of discomfort

in the rearmost upper-right molar.

The tooth in question had previously

received root canal treatment

and a crown at another dental clinic.

On the X-ray,

we were able to identify inflammation

appearing as a dark shadow

below the tooth.

For a more detailed examination,

we took 3D CT images,

which clearly revealed

the inflammation appearing as

a dark shadow at the root tip.

planned to attempt re-root canal treatment

with the aim of preserving the natural tooth.

Given the extent of the inflammation

at the root tip,

we informed the patient in advance

that extraction might be necessary

in the worst case,

and then proceeded with treatment.

We first removed the existing crown.

We then removed the existing filling material

and repeatedly inserted instruments

all the way to the tip of the canal

to carry out disinfection.

After multiple rounds of this process

and confirming that the inside

of the canal was clean,

we filled it neatly with dental material.

In the X-ray images,

you can see that the filling material

appears as a clear white fill

all the way to the root tip.

After completing the root canal treatment

and confirming that the patient

had no discomfort or pain,

we placed a crown

to finalize the treatment. 👍

In addition, at the initial visit,

the patient also reported discomfort

around three wisdom teeth,

including swollen gums and food impaction.

Based on a detailed analysis using 3D CT imaging—

including the position and orientation

of the wisdom teeth

and the location of the inferior alveolar nerve canal—

we thoroughly assessed

the overall oral condition,

and proceeded with careful extractions.

The lower-right wisdom tooth

was lying completely on its side,

so we sectioned the tooth

and extracted it with care.

We have introduced this case

Re-root canal treatment is inherently

more complex than initial root canal treatment.

This is because all of the existing filling material

previously placed inside the canal—

which already has a complex structure

from prior treatment—

must be fully removed,

and any remaining infected tissue

must be thoroughly disinfected again.
